MINOR CONSENT FORMS
A completed and notarized Minor Travel Consent form is required any time a minor child is traveling with an adult who is not their parent — including grandparents, other relatives, family friends, coaches, or group chaperones. Trips booked without this form on file may be delayed or denied at check-in, border control, or security.
WHO NEEDS THIS FORM
You'll need a notarized consent form if the traveling adult is not the child's biological or adoptive parent. This includes:

Grandparents, aunts, uncles, or adult siblings traveling with a minor

Family friends or caregivers accompanying a child

Coaches, chaperones, or group leaders traveling with minors

One parent traveling with the child when the other legal parent or guardian is not present
HOW TO COMPLETE THE FORM

Download the appropriate form below and print it. Digital signatures cannot be notarized, so a physical, printed copy is required.
Fill in the Details
Complete all requested information - child's details, travel dates, and destination, and the accompanying adult's information. Leave the signature lines blank for now.

Do not sign the form yet: The parent or legal guardian's signature must
be made in person, in front of a notary public. Signing beforehand invalidates the notarization and the form will need to be redone.
Bring it to a Notary
Bring the completed, UNSIGNED form along with a valid, government-issued photo ID to a notary public. Sign the form in their presence - this is what makes the consent legally valid.
Return the notarized form
Send us a copy of the fully notarized form ahead of travel, and bring the original along on the trip. Some airlines, border agents, resorts, and event organizers may ask to see it directly.
